Hi Friend,i can only balance the chemical reaction, since before predicting actual and theoretical yields i need how much reactants you have initially taken during the precipitation reaction and finally how much product you have been isolated. Without this information no body can calculate all these calculations.
Co is in +2 state, from given info it should not change its oxidation state since this is not a redox reaction, it's a precipitation reaction. Na is in +1 state. Both these should reflect in products also. PO43- still appear as intact.
