what do you think are some ways we can improve the care we provide to our patients as well as the follow-up our patients have once discharged? You may be wondering why I ask this last part, but lets think about it for a second... if we discharge a patient and they return (or as we call it, readmit), our hospital/health care organization can be penalized for that.
I want to know what you think we need to do to better provide for our patients (not only in the organizational setting, but once they are home)? And how do we make sure that that care follows our ethical principles? And would this post-acute work need to also be overseen by an institutional ethics committee?

yes, we can provide a good tretements even after dischargeby doing daily follow up. After discharging if the patient rejoins then it is all waste of resources so its better to do follow up daily untill they get stable. We can provide better tratement not only in organization but also when they are in home.
1. Better staff with continous updated report about the condition of the patient.
2.Team work and collaboration should be there in this work.
3. Proper authority to supervize the entire procedure.
4. Evidece- Based practice is most important in this case.
With the above we can provide the better treatement for the people.
