Susan is a 65-year-old school nurse who has recently retired from an elementary school. She is beginning to feel rather lonely now that she is no longer working. She is a widow with adult children living out of the area. Her children have suggested that she may want to move to their city and be closer to her grandchildren. She has been avoiding these discussions with her family and tells them all is fine. Moving seems like a major upheaval to her. Although she has been in fair health physically, at her last visit to the health care provider, she had lost 10 pounds and is just below her desired weight.

Nursing Concern and Nursing Interventions:

Nursing Concern: Imbalance Nutrition: less than body requirements, related to loneliness, un-happiness and life-changing circumstances

   Nursing Interventions:

   1. Monitor Susan's food intake daily.

   2. Monitor Susan's weight daily.

   3. Encourage Susan to consume high-calorie foods for patients in her range.

   4. Allow Susan to assist in choosing her meals.

Nursing concerns: Impaired social interactions related to lonliness, Acute low self esteem related to unemployment. Disturb thought process related to life changing circumstances.

Monitoring her weight, diet and intake daily will only focus on her physical state. As she is lonely and depressed it is important to balance her mental state as well. She should be able to interact with someone. Nurses should encourage her to be more open about her feelings. As her childrens suggested her moving to their city, explain her that change is constant. In simple words Nurses should give her counselling and guidance.

Nursing interventions:

1. Encourage Susan in meditation.

2. Encourage Susan to interact with her family members.

3. Encourage her to follow her diet strictly.
