A 25-year-old Arab American man is one day post op following a thoracotomy. There have been no post-surgical complications. He tells his family that he is “in terrible pain”, yet he reports ratings of 2 to 3 on 0-10 numeric rating scale to the nursing staff. He requests pain medication every two to three hours, and will not get out of bed or ambulate.
Scenario 2
A 60-year-old African American woman with metastatic ovarian cancer is experiencing chronic pain. Her pain ratings have been between 3 and 7 on the 0-10 numeric rating scale. Her vital signs remain within normal ranges. She is fully able to participate in her daily care. She describes herself as the matriarch of her family, and looks forward to daily visits from her family even when her pain rating is high. Her church pastor calls and visits her several times per week. She anticipates discharge in a few days.

Explanation / Answer
1. There is a huge discrepancy between the two patient's pain rating because pain score is a subjective data and each individual can have the different perception of their pain. Moreover, every individual is unique and their pain threshold is also different. That is why the pain score which is too low for the first patient may be too high for the second lady.
2. The two of them showed entirely different behavioural signs which are not coinciding with their with their pain score. The behaviour the man is showing clearly indicate a high level of pain but his pain score is low because his tolerance power is more and he can even bear more with is willpower whereas, the lady's behaviour is completely not in favour of her pain score.
3. The lady has a very good participation in her daily care and treatment because she has overcome her condition and indulge her self in recreational activities which indicates she is doing good and ready to be discharged whereas the man still has severe pain which is hindering him to participate in the activity of his daily care and treatment.
