Bariatric surgery : it is a weight loss surgery , where a portion stomach or section of constricted by a band or small intestine is resected or diverted re routed .

Weight loss is due to the restriction/ decrease of nutrient intake that is created by the small gastric pouch. Where less food is absorbed leading to weight reduction .

International diabetic federation has stated that client with / more that 30-35 kg/m2BMI are eligible for bariatric surgery . hence our client jillian is eligible for bariatric surgery ,but she has to undergo psychiatric clearance for the surgery as many clients with /more than 40 BMI has depression .

Post surgery diet modifications:

Immediatiely post surgery :

1)      Jilian is restricted to a clear liquid diet ( clear broth, diluted fruit juices or sugar-free drinks and gelatin dessert)

This diet might be continued   until the gastrointestinal tract has recovered from the surgery.

After the recovery from surgery (after 2-3 days )

The next stage will be with blended or pureed sugar-free diet (two weeks).

It is of high protein, liquid or soft foods #protein shakes, soft meats, and dairy products.

Later stage – soft diets are recommended. porridge, smashed potatoes .

Client recommendations : 1) eat small frequent meals 2) drink sips of water throughout the day to prevent dehydration 3) take soft ,tolerable food 4) try food slowly one at a time 5) maintain a health menu to prevent complications of malabsorption /malnutrition 6) recommended to take vitamin/mineral supplements .

Foods high in carbohydrates are usually avoided initial weight loss period as the stomach portion is reduced the body has to accustomed to the quantity of food we eat , avoiding food which might causes distention are avoided causing distention of stomach may stress the surgical wound and stomach .

Client is recommend to take high protein diet to enhance wound healing and maintain the body mass in later stage due to malabsorption the body loose drastic amount of weight . client may resume normal diet after 3-4 months after surgery , depending upon where (stomach / small intestine the surgery is performed and to the tolerance of the individual client .

Clients has to avoid

high fiber rich food which cannot be digested /absorbed completely causing bowel irritation .

high sugar, nuts , fried items tough meats which might need lot of digestion .

Dehydration is frequent after bariatric surgery has the stomach pouch is being reduced so the client can not drink to her recommended water level   and less water amount absorbed (resection or diversion of small intestine ) leading to dehydration , as signs of dehydration client will has symptoms of vomiting and diarrhea . so the client is advised to take 45-60 fl oz which is 1.2-2 l of fluid in sips throughout the day .

Dumping syndrome is rapid emptying of the stomach which is usually seen after surgery is due to increased movement of stomach . it is seen more with sugary food and larger meals . it is advisable to avoid simple sugary food and take smaller meals frequently .it extreme cases medications may be needed .
