A 7-year-old girl has been admitted to the pediatric ICU for sepsis. The sepsis was caused by a Streptococcus pneumoniae infection of the RLL. Currently, the client is confused and has increased respiratory rate, nasal flaring, scapular indrawing, increased heart rate, decreased urine output, and hepatomegaly. The client, who has severe asthma, has recently received several courses of high-dose corticosteroid therapy. The client’s immunosuppression was the causative factor in this case. Explain.

Long term corticosteroids may lead to immunosuppression and may resulting in occurance of other disease . patient immunity will be poor in this case and they cannot fight against infection. Many disease, especially respiratory in origin may occur for this patient easily and Pneumonia is one amongst them . For a patient with asthma need corticosteroid therapy for long term . In this case it is important to monitor the dosage and duration of the course. By following strict aseptic techniques while handling equipments , educating patients and by maintaining hygiene it can be prevented by some extent . The patient should limit visitors and try to wear protective equipment like masks if necessary . Prophylactic antimicrobial agents also needed for to avoid further infection. In this case girl is infected and her condition is worse . Close examination and chest X Ray is indicated to find out extent of disease and She needs oxygen therapy, IV fluids, antibiotics and continuous monitoring . A nurse should take precaution to prevent any other infection for this girl.
