Question
You are the Request of Release of Information (ROI) Coordinator. You are having a busy day today and are facing a variety of ROI situations. How should you handle the situations described below and what, if any additional information will you need? If you will need to provide copies of the medical record, explain which forms (information) you would provide. Include dates if appropriate. 1.A patient just called. The patient has an appointment at her doctor’s office in 1 hour and wants copies of records to take to the office. The hospital has a policy that requires a 48-hour notice, except in medical emergencies. The patient did not specify what records she needs. 2. A patient has requested that his charts be sent to Disability Determination 3 times and that department has not received them. Your records show that they have been sent twice. The patient is very upset. 3. An FBI agent shows up and flashes a badge at you. He demands that you release a patient’s chart to him immediately.
Explanation / Answer
1. With patient id details of her treatment can be collected. Reverification of the specific details related to her follow up treatment can be made by making call to patient and enquiry about her reports which she want need to collected. The details about her recent reports with id can be collected and patient documents can be handed over to her via mail or giving a hard copy of the documents when she arrives for her appointment.
2. A telephonic conversation can be made between disability determination of the office and ROI that can be recorded as a proof for further clarifications. Even a personal letter can be sent in order to verify about the receive documents.
3. As a ROI I need to verify about FBI officer identity by verifying their documents and then I can track the patients id about whom they want details. I can give specific details after reassuring about their identity and patient details by following the guidelines of our healthcare system, as each organisation follows certain guidelines and legality before handing over patients details.
