Question

Scenario #1: You are the nurse working on a telemetry unit. You have been assigned to take care of Mr. Fred Smith, a 49 year old admitted to the hospital for a cardiac catheterization. During your initial assessment, you ask Mr. Smith why he was being admitted. He states, “to have a heart test.” He is unable to explain further and you note he has a knowledge deficit regarding this procedure. The next morning you notice that Mr. Smith is being taken out of his room and sent to the cath lab. You inquire whether the physician or anyone else had visited with him to discuss the diagnosis and the impending cardiac procedure. No further discussion occurred. You note the consent form has not been signed. As you talk with Mr. Smith you note he still does not know much about the procedure and states, “I know what I need to and that’s to leave it in the good hands of the doctor.” The physician asks you to witness the consent form immediately as they are backed up on cases and want to get Mr. Smith’s procedure done.

1)What is the role of the nurse in this situation?

2)Can a patient avoid informed consent?

3)Would you witness this consent form?

4)What strategies would you utilize to talk to this patient? To the physician?

Explanation / Answer

3. In an informed consent, the nurse's role is both as a witness and as the patient's advocate. So she can witness this consent form.

4. The strategies that I would you utilize to talk to this patient are supporting patient comprehension by involving with permission patients' families and significant others in the patients' care.
