bone infection. He is treated with clindamycin by intravenous infusion (EMC, 2014c). According to the BNF, the dose of clindamycin is 0.6–2.7 g daily in two to four divided doses. e medical team has prescribed 600 mg four times a day.

Your colleague has found the product supplied by the pharmacy, which is clindamycin 150 mg/ml injection in
4 ml ampoules, and plans to draw up the contents of 4 ampoules in order to give the prescribed dose.

Finally, you need to check the dilution. Appendix 4 of the BNF says clindamycin should be diluted to a solution containing not more than
18 mg/ml with normal saline or glucose 5%. It must be administered at a rate not exceeding 30 mg/minute. Your colleague intends to add the dose to 250 ml normal saline and to set the IV pump to give the infusion over 15 minutes.

Dilution and infusion rate of clindamycin Drug:

1ml contains 150mg of clindamycin.

4ml ampoule contains 600mg of clindamycin.

Therefore the medical team recommended Mr. Frederick is 600mg four times a day.

The concentration of clindamycin dilution for infusion should not exceed 18mg/ml. The infusion rate should not exceed 30mg/min.

The usual infusion rate is

To administer 600mg of clindamycin each time the drug to be diluted as

4ml in 50ml of NS

so 54ml contains 600mg of clindamycin.

therefore 1ml contains 11.1 mg of clindamycin.

If it administered 54ml over 20min is equal to 2.7 ml/min.

That equals to 29.97 mg/min. (it does not exceed 30mg/min).

The dilution is mixing of the 4ml ampoule in 50ml NS and administered over 15 min.
