The telehealth services has changed the face of the traditional health delivery system from the past few decades. It

is used for rendering clinical services as well as for providing educational services.

The increase in growth of Geriatric population, the lack of adequate medical practitioners in the areas, cost-

effectiveness of treatment, a fast delivery of result etc, has favoured the wide popularity of telehealth services

among the population.

To support this, a study was conducted by Global forecast- 2021, which reports an expected increase of

telemedicine market from 2.75 billion to USD 9.35 billion USD by 2021.

Telemedicine is a technology for “real-time” interactive communication, providing remote health care services to the

patient, through the mode of electronic audio or video services.

The certain components such as hardware (like glucometer, sphygmomanometer, weighing machine etc),

software( to support a flawless video and audio interaction), end users ( patient, provider or payers) and the

mode of delivery of service( cloud/web) all integrate the success and possibility of a powerful telehealth

services providing door to door service.

The Center of Medicare and Mediaid services ( CMS), pays for the services under certain carefully defines

conditions

- the beneficiary should be located in a Rural Health Professional Shortage Area (HPSA)

- the country located outside the Metropolitan Statistical Area (MSA)

- the service site should be authorised by law

- the service provider must be authorised by law

- there should be a real-time interactive service between the patient and the provider

- submit the claim using the appropriate CPT /HCPCS Code along with the modifier GT

- a payment is received to the originating sites for delivering telehealth services by HCPCS code Q3014

How can the new changes affect the current Telehealth Coding?

- The new code "POS O2" code takes the place of "POS code"

- The Center for Medicare and Mediaid Services (CMS ) states the latest rule, the provider should indicate

that the service was furnished from a distant site.

- Use the CPT/HCPCS code along with the modifier GT. For example, if you want to report the office or

other outpatient visits..use CPT code 99201-99215 GT.

- The receiver should be present at the site where you have provided the service, at the time of adding

modifier GT to the code.

- In case of adding GT to ESRD- related service code, provide one hand-on visit on “vascular access site”

in every month.
