While assessing J.D.'s vital signs at 12 noon, you note that there is no urine in the tubing and there is only a little in the bag. Explanation / Answer

1. I think the catheter misplaced/dislodged from the actual position or clamped for the assesment of patient for the catheter removal. because it's a 2nd postoperative day patient mostly mobilized and they can do their daily activities with support, on other hand if it can misplaced/dislodge due to the mobilization.

2. in this situation i first asses the patients drainage system for any occlussion or the clamping. this because the output is less and patient feels to urinate.

3. For this i will consider her condition, if she mobilizing and doing her activities self or with minimum support, i will remove the catheter. if the client not doing activities self with minimum support, i will reposition or reinsert a new catheter.
