1. The process that is used to assess, plan and teach family members as well as patients by me as a caretaker was as follows:

For the assessment we need to know as much about the family as possible. We need to know who all are considered as part of the family as well as the role and position of patient/client in the family. We need to assess age, sex, health status and occupation of family members. Then we need to determine which family member has the most influence on the patient.

Now we need to know about the resources that are available to the patient. These include if the home of patient can give proper safety and comfort and what is the level of patient up to which he/she can perform self care in this home. Then comes the health insurance resources and whether the community resources are available for the patient or not.

Information about the family's emotional beliefs, education, willingness to learn about care giving, normal diet routine is also obtained for this assessment. Then we need to observe if the family members are sensitive to the patient as well as each other. How effectively the family takes a decision as a unit is also an important point for assessment. Then comes the family's understanding of the current health problem. Also what major problems the family has faced since the illness occurred in the patient.

Finally the patient's as well as family's teaching needs are assessed. Are the family members familiar with someone with the same problem or not. Finally it needs to be determined if the patient and family members agree to the treatment plan outlined. Also information regarding any barriers to learning is obtained.

Once we have assessed the family we need to plan a method by which we will teach the members. The strategy to teach is given as follows:

One of the methods to teach is the COPE model. This model is very effective in helping family members of patient to become effective problem solvers. C is for creativity. Creative ideas that can help the family members to overcome any kind of obstacles that come in carrying out proper care and management. Thus the problem has a creative solution. O is for optimism. In optimism the emotional aspects of patient's needs are given a priority. It helps the caregiver in having interpersonal skills that improves the communication of the patient and care giver. P is for planning. In this one learns about the how to plan for future problems in the family. Planning for contingency plans when one plan fails is also advisable. E is for expert information. In this members of the family are provided background information. Apart from teaching the family members what to do if we teach them the reason for care tasks specifically then it improves the overall quality of care the patient gets.
