How might age and developmental stage make a difference in ur assessment of a patient? Suppose you are the triage nurse at the community clinic in the Meet Your Patient scenario.What kind of assessment do you perform at Sami's first visit (initial, ongoing, comprehensive, focused)? Initial assessment Example:
    In emergency department as soon as patient comes initial assessment by general apperance checking by thier skin,colour anf full general examination..if the patient present with normal skin and dehydration with conscious level and one more patient with grey skin and sweating need aggressive assessment and treatment..Initial assessment we will in emergency room is patient history,general appearence,physical examination and vital signs..
Ongoing assessment:
    If we findout anything abnormal in vitals,we need to check the vitals every 2 hours for clear decision making process..In emergency department if patient having fluctuate BP we need to check BP every 15 minutes ..Ongoing assessment we will do every 15 minutes for stable patient and every 5 minutes for the unstable patient..
Comprehensive assessment:
   We will do this assessment when patient come to emergency with complait without any previous health records...We need to do complete assessment and detailed history and making some investigation for the patient to role out their problem..Patient when they come to Emergency with complaint of breathing difficulty and palpitation we need to do complete assessment physically and psychologically what causes this condition..
Focused assessment:
   this is when patient come to Emergency department with trauma and conscious level..We will assess chief complaint ,past medical history,present signd and symptoms with vital signs checking and sample history like they have any allergy for any medicine and last oral intake and all..
      Assessment can be done by varity of professional with different settings..it is a methodology foe human development,each stages also habits own strength and vulnerabilities...whn planning,designing and implementing the assessment for each individual as per their age group like childhood,young,middle adulthood,older adulthood to meet the health related needs by developmental approach must be used,,Because this age group having different physical,congnitive and psychological maturation and health ability to cope with their illness..
   As a Triage nurse i will do focus asessment by rapid history,complete vitals and sample history collection from family or friends to know about physical condition..Like patient allergy condition,medical histroy,incident when this injury occured..whether the patient is in nil per oral or not..
