He has been in and out of psychiatric facilities for several years. He left the reserva for schizophrenia at age 21 Case Study: I 5 Dean has come to the community health center. While he is siting in the lobby,the nure observes Dean without him being aware of this. The nurse notices at times he seems to be talking to the air around him. She also notices that Dean is smacking his lips, his tongue protrudes at times, and he periodically coughs without covering his mouth. When it is time for the nurse to do an assessment on Dean, the receptionist notifies the nurse that Dean has not filled out the intake form. The nurse tries to help him by asking him his address. Although his speech is somewhat disorganized at times, the nurse finds out from Dean that he is homeless and currently living on the streets. His chief complaint is "not feeling well."
