WHat is the service code and icd 10 cm?-------------------------LOCATION: Outpatient, Hospital PATIENT: Peggy Twite PHYSICIAN: David Barton, M.D. RADIOLOGIST: Alfa Aljabar, M.D. EXAMINATION OF: Stress and rest myocardial perfusion scan CLINICAL SYMPTOMS: Shortness of breath; status postop pacemaker STRESS AND REST MYOCARDIAL PERFUSION SCAN: TECHNIQUE: 29.5 millicuries of technetium99m tetrofosmin was administered following stress with adenosine, and 26.5 millicuries of technetium99m tetrofosmin was administered at rest. SPECT imaging was performed in all three planes. FINDINGS: Ejection fraction is prominently decreased at 12%. On the previous study, it was 21%. On the static images used to calculate ejection fraction, there appears to be dyskinesis of the cardiac apex, the inferior wall, and portions of the lateral wall. There is dilatation of the cardiac chamber. Decreased myocardial perfusion is seen in the inferior wall, with only minimal reversibility. Decreased myocardial perfusion in the cardiac apex extends into the periapical anterior wall, and this is a mixed myocardial perfusion defect, with some reversibility. There is thinning of the lateral wall and septum, but these are not significantly reversible on rest images, and the septum actually appears somewhat worsened on rest images. IMPRESSION: 1. Predominantly fixed myocardial perfusion defect involves the inferior wall, though there is minimal reversibility. 2. Mixed myocardial perfusion defect involving the cardiac apex and periapical anteroseptum. 3. Mild thinning of the lateral wall and more prominently the septum, which is not reversible. 4. Dilatation of the cardiac chamber. 5. Prominently decreased ejection fraction at 12%. Please see above comments.
