A case study: A terrible collision between a trailer truck and a bus has occurred on Route 51. Several of the passengers are rushed to Cannonsburg Hospital. Upon examination of Mrs. Washington, a 45 year old woman, the following notes were added to her chart.

Fracture of superior region of right tibia (shinbone of leg); skin lacerated; area cleaned and protruding bone fragments subjected to internal reduction and casted.

Nutrient artery of tibia damaged

Medial meniscus (hyaline cartilage disc) of right knee joint crushed; knew joint inflamed and painful.

QUestion: What type of fracture does Mrs. Washington have and what are the problems that can be predicted with such fractures and how are they treated?


Explanation / Answer

When the bone punctures the skin in a fracture, it is called a compound fracture (also known as an open fracture). Compound fractures carry a high risk of infection since an open wound is involved. If an infection develops, there can be problems with recovery and bone healing. In extreme cases, gangrene can develop resulting in amputation or death. To avoid infections, compound fractures are often surgically repaired and sealed. Antibiotics are also administered to kill off any bacteria that may have gotten into the wound.
