1)Binding to plasma proteins plays a major role in drug therapy because it affects pharmacokinetics and pharmacodynamics of drugs.Protein binding of antibiotics may affect the efficacy of therapy in following ways-

A)Only the non-protein-bound fraction of a drug in plasma can penetrate into and make space in the extravascular space. Penetration into the extravascular space is very important for antimicrobial therapy, as the majority of bacterial and fungal infections occur in the extravascular space like interstitial fluid of tissues .

B)Protein binding also affects drug clearance by tubular secretion or hepatic metabolism, high PB is associated with lowered drug elimination and it negatively correlates with glomerular filtration, due to negatively charged protein repulse negatively charged glomerular membrane so only the free drug is filtered.
