Friends and family report these recent behaviors as “totally unlike her,” describing her as a normally conservative, responsible person. Her cousin becomes concerned and accompanies the woman to a Crisis Unit where she is subsequently diagnosed with Bipolar Disorder and admitted to a Mental Health Unit. Address the following: What is the priority RN action for this client? Which medical concerns may be significant, considering this client’s recent behavior? Discuss what the RN would teach the client about Bipolar Disorder. Provide two nursing diagnosis statements (each statement must include an actual nursing diagnosis, related factor and as evidenced by) that might apply to this client.

(1)What is the priority RN action for this client?

The main thing which should be considered is the patient safety. Patients who are suffering from maniac or depressive phase of bipolar disorder tend to show signs of suicide and so one should make sure the patient or client should be safe and for further treatment, he should get admitted safely in the hospital.

(2)Which medical concerns may be significant, considering this client’s recent behavior?

One important thing you need to remember is the persons who are suffering from bipolar, will be hyperactive and will not take food properly, they will not sleep well due to these reasons many times they feel exhausted and will look malnutrition.

(3)Discuss what the RN would teach the client about Bipolar Disorder.

The RN would tell them, that it is a chronic mental illness, and people with this disorder will have mood swings. They can be related to depression to mania. Sometimes they may feel hyperactive, suddenly they may enter into depression, that is the reason why we need to keep them in safe environment, always they must be under psychiatric follow ups, it is must for them to take medications to keep their mood swings in control, along with that they should go some recreational activities, family should support them in all aspects.
