If your blood has the protein, you're Rh positive. If your blood ( lacks the protein, you're Rh negative. If a woman who is Rh negative has an Rh positive baby and becomes pregnant with a second Rh positive baby, her body can attack the second baby's red blood cells. Why is this only an issue with the second pregnancy?

1. Antivenin is nothing just the antibodies against the venom of the snake. Antivenin at that time neutralizes the snake poison in our body and we are good at that moment. but that antivenin acts as antigen for our body and our body prepares antibody for the same. Now when second time it will be inserted into our body our body is already ready with its army of soldiers( antibodies) to attack it. Now doctor won't give you again because there are high chances for an immune response and for your life being at risk.

3. The mother is Rh negative. At the first time when she is pregnant with Rh-positive child. Some of the Rh antibodies either gets transported through the placenta or come in contact with her body during delivery. So her body prepares antibodies for Rh and when she will be pregnant second time with Rh-positive child again then these antibodies will travel through the placenta to her womb and will kill the foetus. This is known as erythroblastosis foetalis.

This happens only for the second time as first time the body just prepares the antibodies for Rh. And second time only it encounters the Rh and thus an immune response is initiated.
