She is majoring in biology and hopes someday to be a pediatrician Beginning about a month ago, Jamie noticed that she was waking up once, sometimes twice a night, by the need to go to the bathroom. More recently, she has noticed that she needs to go to the bathroom during her school day much more frequently than before, sometimes as often as once every hour At first, Jamie thought that her increased frequency of urination was due to all the coffee she drank, but when she reduced her coffee consumption to one cup in the morning, she still needed to go to the bathroom just as often. She also noticed that her urine seemed pale and colorless When Jamie told her mother or her problem, her mother became very concerned and arranged for Jamie to see the family physician. Her physician found no abnormalities on physical examination. However, a blood chemistry profile revealed the following Plasma Plasma osmolality 308 mOsm/kg sodium level 149 mEq/kg Fasting plasma glucose 85 mg/dl Urine osmolality 200 mOsm/kg Urine glucose: negative

Explanation / Answer

8. Based on the results of MRI, Jamie's post dehydration osmolality(mOsm/kg) of plasma and urine are 298 and 200 respectively. Post DDAVP values are 290 and 815mOSsm/kg respectively. By these results, it can be concluded that Jamie is having the condition of cranial diabetes insipidus. The condition is usually caused due to damage to the pituitary gland or hypothalamus. This may result in disruption of the normal production, storage and release of ADH. The damage may be due to surgery, a tumor, or meningitis, inflammation or a head injury. Children may be affected with the condition due to genetic inheritance. Sometimes, the condirtion is resulted by unknown causes.

9. Jamie is 20 year old junior student is affected with frequent urination or polyuria and polydipsia (increased thirst). Physician carried out diagnosis by conduting the medical tests such as fasting blood glucose, plasma sodium, plasma osmolality, urine glucose levels and glucose tolerance test. The plasma and urine osmolality was measured post dehydration and post DDAVP. The values obtained as post dehydration osmolality(mOsm/kg) of plasma and urine are 298 and 200 respectively. Post DDAVP values are 290 and 815mOSsm/kg respectively. The results indicate that Jamie is having the condition of cranial diabeted insipidus (DI). The condition is treated by desmopressin given as intranasal or oral tablets. Carbamazepine is given as an anticonvulsive medication, had shown success to some extent in this type of DI.
