What would be the practical consequences on the fast-food industry and on customers if cashers generally shortchanged customers at every opportunity?

Bottom line is that people would stop buying from places where they are continually short changed. The person being short changed may not notice it at the moment, but most will realize that something was wrong later when they go to make another purchase and they do not have as much money as they thought they should have. This practice is criminal, when done intentionally. Yes, we all make mistakes at times, but to short change intentionally someone we suspect is not paying attention, is stealing.
