Question

The patient just graduated college with a degree in Mechanical Engineering and is working full-time for the Department of Transportation. He is 25 years old, diabetic and is lactose intolerant. His blood pressure is 134/91, he weighs 225lb, he is 5ft 9in tall and his waist circumference is 44in. He had a cholecystectomy when he was 24 years old and has experienced since then an increased number of bowel movements and some gastrointestinal discomfort. He also expressed that he does not eat dairy products in general but he usually eats two containers of yogurt every day (see food label below). When asked about his daily physical activity, he admits to only walking around his neighborhood maybe once a month. He keep a record of his diet for a week and these are some of the results: 3425 kcal/day (total intake), 1321 kcal from lipids, 250 kcal from protein and 1854 kcal from carbohydrates. Answer the following questions and make sure that the answers are backed by quantitative data when available.

Is he overweight or obese? Why?

What method would you choose to estimate BFP if it’s necessary? Why?

Is he eating more or less than he should? Why?

How many grams of protein should he ingest on a daily basis to maintain his current weight?

What are some of the problems with his current diet? (be specific and backup your answer with data) What changes should he make to help him lose weight?

What percentage of his daily value of protein does he get from eating yogurt?

Based on what you know about him, why do you think he eats yogurt every day?

What do you think is the cause of his increased frequency of bowel movements and gastrointestinal discomfort he has experienced since his surgery?

Explanation / Answer

1. Is he overweight or obese? Why?

His weight is 225lb (102.058 kg)

Height is 5ft 9in = 69 in

Waist circumference is 44in.

Metric BMI Formula

BMI = weight (kg) ÷ height2 (m2)

Imperial BMI Formula

BMI = weight (lb) ÷ height2 (in2) × 703

Patient’s BMI = 225÷ (69 x 69) x703 = 33.23

BMI = 33.23 kg/m2   (Obese Class I)

Normal BMI range: 18.5kg/m2 - 25 kg/m2

Waist circumference more than 37.6 in is considered unhealthy and obese.

2. What method would you choose to estimate BFP if it’s necessary? Why?

BFP can be calculated from Age, Weight, Height, Neck circumference, Waist circumference, Hip circumference

Body Fat percentage = (Total body weight x 1.082) + 94.42

3. Is he eating more or less than he should? Why?

Harris-Benedict equation to calculate daily caloric requirements

For men, the formula is: 66 + (6.23 X weight in pounds) + (12.7 X height in inches) - (6.8 X age in years).

i.e. 66+(6.23 x 225) +(12.7 x 69) – (6.8 x 25) =

66 + 1401.75 + 876.3 – 170 = 2174.05

From the patient's records, he is consuming 3425 kcal/day (total intake), 1321 kcal from lipids, 38.6 %

250 kcal from protein 7.2%

1854 kcal from carbohydrates 54.1%

4. How many grams of protein should he ingest on a daily basis to maintain his current weight?

The patient must get 12.5% energy from protein

Intake must be 0.8g per Kg body weight

i.e. 0.8g x 102.058 kg = 81. 64 g

5. What are some of the problems with his current diet? (be specific and back up your answer with data) What changes should he make to help him lose weight?

From the patient's records, he is consuming 3425 kcal/day (total intake), 1321 kcal from lipids, 38.6 %

250 kcal from protein 7.2%

1854 kcal from carbohydrates 54.1%

He should follow the ideal calorie intake to lose weight, should be like 40-60 % Carbohydrate

30-40 % protein

10-20% fat

He should increase his physical activity too.

6. What percentage of his daily value of protein does he get from eating yogurt?

1 serving of yogurt has 11 g of protein

So he is getting 22 g from 2 cups

His daily requirement of protein is 81.64 g

So he is getting 27.02 % from yogurt

7. Based on what you know about him, why do you think he eats yogurt every day?

The lactose content of the yogurt is also less compared to other milk products.

He may be eating yogurt to ease the lactose intolerance and the GI discomfort.

8. What do you think is the cause of his increased frequency of bowel movements and gastrointestinal discomfort he has experienced since his surgery?

Cholecystectomy is the surgical removal of the gallbladder.

The patient may experience difficulty in digesting fatty foods after gallbladder removal. The patient’s increased fat intake may be the reason for the bowel discomfort.
