Ap Surgical Case #15 Operative Report Procedure: Diagnosis: Anesthesia: Technique: Right carotid thromboendarterectomy and vein patch angioplasty Right carotid artery stenosis with hemorrhagic plaque General Under general anesthesia, the right side of the patient's neck was prepared and draped in a sterile fashion. An incision was made along the sternoclei- domastoid muscle, and a sharp dissection was carried down to expose the common internal and external carotid arteries. Care was taken to avoid injury to the hypoglossal, vagus, and ansa cervicalis nerves. The vessels were insert- ed with elastic tapes. The patient was heparinized, and an arteriotomy was made. A very high-grade stenosis estimated at 1.5-mm opening was present at the origin of the internal carotid artery, and there was evidence of significant hemorrhage and degeneration in the plaque. A shunt was placed for cerebral perfusion, and then the endarterectomy was done in the usual fashion. The vessel was picked free of any debris or other plaque and then closed with a patch angioplasty using saphenous vein from the left groin. This was sutured in place with 6-0 Prolene. Flow was first directed into the external carotid to low any possible air or debris to escape and then restored to the internal wound was packed open until the end of the procedure and after parin was reversed.
