NAMES: A 25 yr old man has just returned from a Central American mission with the Peace Corps. He says he is having pain in the upper right region of his abdomen and he has a fever. He claims that he experienced a bloody and mucousy diarrhea about 2 months ago. A CT scan of his abdomen shows an abscess in his liver. Aspiration of material from the abscess was performed. Microscopic examination revealed motile, non-flagellated trophozoites with ameboid movement. 1. a. What is the causative agent of his infection? b. How did he likely acquire the infection? c. How does this organism lead to bloody diarrhea with mucous?Explanation / Answer
a. The most probably causative agent of his infection is Entamoeba histolytica . It causes amoebic liver abscess.
b. The infection of Entamoeba occurs via faecal-oral route.
So it is likely that the old man has taken food or water contaminated with cyst of Entamoeba.
C. The E.histolytica is invasive in nature. Its trophozoites penetrate the columnar epithelial cells in the crypts of liberkuhn in the colon.
- mucosal penetration by the amoeba produces discrete ulcers. Moreoften , the amoeba penetrate the submucosal layer and multiplies rapidly , causing lytic necrosis and form abscess, which break down to form an ulcer.
