This capstone project tracked patients who were newly diagnosed with diabetes at a large health system. When patients are diagnosed with diabetes at this health system, they are emailed instructions at discharge asking them to call and make a follow-up appointment with a diabetes specialist team to develop a care plan. However, many patients did not do this follow-up. So when they came back for their annual visit, many had high levels of hemoglobin A1C (HA1C), which is a marker of poor clinical health and poor health care quality (at least to a degree).
Before going to senior leadership, department managers at this health system wanted to try to improve outcomes using simple tweaks to their processes. Their twin goals were (1) improving the rate of newly-diagnosed diabetic patients who made follow-up appointments and (2) lowering HA1C levels across the patient population. Theoretically, achieving goal #1 should lead to achieving goal #2, but the group wanted to check for both separately. The strategy they had for achieving these goals was to overhaul the instructions emailed to patients, then measure whether patients were more likely to make follow-up appointments to see their primary care provider and measure whether patients’ HA1C levels changed after 1 year. Each patient was randomly assigned into one of 4 groups: group 1 (control group, received the current email [generic text sent from donotreply@hospital.org email address), group 2 (received standard email text, but email was sent from their personal doctor’s email address), group 3 (received newly revised email text, with email sent from donotreply@hospital.org email address), group 4 (received newly revised email text, with email sent from their personal doctor’s email address)
