Explanation / Answer

Handoff report is the report about the patient's condition including the information regarding medications,diet,intake and output, what all procedures where done , vitals etc which are communicated by the previous shift nurse to the next shift nurse . It is the brief summary of what all the previous nurse has done for the patient in the last 8-12 hrs.

Handoff report is very important and has a major role in the patient's improvement , safety and early discharge.

In a handoff report we are transferring the responsibility and accountability of a patient to another health care staff.

Effective communication is the essential part or the backbone of handoff report . When we are transfering information it is basically we are communicating . Ineffective communication can lead to many complications like for example haven't informed about the medication given and the next shift nurse administers the medicine second time again ,leads to double dose of some high risk drugs may cause fatal complications etc. Ineffective communication in the handodd report occur by the following ways:-

-The handover report is not complete.

-Inadequate information regarding the procedure or actions to be done for the patient

-Language problems

-Unable to recognize the writing due to poor quality of handwriting (what is written in the chart etc)

-No written documentation, more rely on memory

-Poor patient involvement.

-Multiple staffs involved in the care of the patient.

For the effective communication during handoff report we must have standardized structure of communication which should include the components- that whom should provide handoff report, what specific information to be transferred, how much time should be taken , the surrounding environment during communication of handoff reports .

Handoff reports must be short and accurate . The written information about the patient must easy readable and of good quality , legible ,no over writings, cuttings.

While verbal communication use a language which is understood by both the medial staff and patients .Minimise interruptions, and different slangs.

Set time for communicating the handover report to avoid unnecessary stretching and surrounding environment should be calm and quiet moderately to avoid noise barrier.

For effective communication currently the health care professionals are using the SBAR - situation, background, assessment and recommendation technique .

Not only communication , leadership also plays a important role in effective handover . Leadership is to guide a person through a right track to attain a goal /aim .

The health care professionals must be led by a good leader through right track using standardization, effective communication, to attain their goal of effective handoff reports.

The supervisor should supervise and train the staff about handoff report . Periodic evaluation of handoff reports must be done to improve the level . No bias against any staff members , everyone should be treated equally . By the use of effective communication and leadership tools we can have a effective handoff reports which will help in safety ,health improvement of patient.
