Design a questionnaire that you will use to survey at least five potential customers to determine the level of interest in your product or service. Your survey must include a description of your product or service, and you must provide a minimum of five questions in your survey. Potential questions to consider include: What do you purchase now and how do you decide what to buy? Are you satisfied with the current product/service offered in the current market? Will you be interested in the new product/service that we are offering? How much will you be willing to pay for the new product/service? Prepare a summary of your survey results that addresses possible decisions that could be made regarding your product or service. Be sure to include the questions used in your survey and the responses to the survey. Do not include any personal information.

Design of Questionnaire for survey

We know that we need to prepare the well organized questionnaire for collecting the responses from the respondents under study. For the collection of the information or data from the respondents we need to use the questionnaire which is easy to understand. Also, the language of the questionnaire should be clear and responder should know this language. The order of the questions in the questionnaire would be proper such that the responder would not be confused during submitting the responses. Also, sometimes we do not get the responses for some of the questions because responder is not reliable or confident or responder would hide some information due to some reasons. For the given scenario, the five questions related to new product survey are given as below:

What do you purchase now?

How do you decide what to buy?

Are you satisfied with the current product/service offered in the current market?

Will you be interested in the new product/service that we are offering?

How much will you be willing to pay for the new product/service?

For the above questions, the possible alternatives should be available for giving response. Also, sample size for the collection of data should be adequate because sometimes we get missing data or non-response from the responder. All care should be taken while collection of the data by using the questionnaire.
