The mann whitney U test is used to measure difference in means. So this is not the correct test.
In a paired t test data is obtained from study subjects before and after a study, so there are 2 data points for each subject here. This is a possible test here.
In an Unpaired t test, the data groups are independent. This is not the case here, as both groups are sufferring from the same problem.
Chi square Test is used to calculate whether there is significant association between 2 variable (eg age groups and the brews they prefer). In this test we are not searching for association.
Therefore the paired t test is the correct answer.
