Question

can someone help with the flow of the paper, grammar, and punctuation, and any suggestions to make it sound better?

After completing the assessment, I have determined that my patient has mild degree of impairment based on scoring a 25 on the MMSE test. Reviewing the interpretation of MMSE scores, my patient scored between 20-25, which indicates mild degree of impairment. after reviewing his medication, current medical history, past history, I found that he has a Pituitary tumor. My patient was in a MVA due to having eye sight problems from his Pituitary tumor. He says his Pituitary tumor causes him to have blurred vision which impairs his driving; especially at night. In the MVA he sustained multiple broken bones throughout his body, and he fractured his skull. When I spoke with him, he told me he fractured his skull which left him speech impairment. He said he had speech therapy which helped with his speaking ability. When talking with him I noticed his speech was fine, not slurred, or delayed. He told me that because of the accident it difficult to know what day it is because he’s been in and out of the hospital for months for surgery and rehabilitation. Even though my patient did well overall, I believe my patient has some impairments based on his medical history , Pituitary tumor, and the injuries he sustained in this MVA. I believe because he has a Pituitary tumor it causes him to have blurred vision which affects his cognitive ability visually to see what month, day it is on a calendar, the time on a watch.
