She states she was picking flowers in her garden and that when she stood up she felt a sharp pain on the right side of her lower back. She is concerned because the pain hasn't gotten any better over the past few days and she is unable to get comfortable. Explanation / Answer

1. Based on the information provided the patient may have pain due to sudden muscle pull or spasm while standing up from bend position.

2. In this condition sciatic nerve may be affected. It arises from L4-s3 segment of sacral plexus.

3. Muscles which can be tested are piriformis , group of hamstring muscles,calf muscles

4. We can ask the patient to come in supine position ( according to muscles which we want to test). If the patient is not comfortable in supine we can also test the muscles in side lying position.

5. After testing there can be, tightness of the piriformis, hamstring or other muscles of the posterior region of the leg. Tight muscles usually compress the nerve passing under the muscle.

There can be altered biomechanics.
