Potential ethical violation in this case is of harming the patient by trying to extract more from them. The psychologist is already seeing that Medicare doesn't cover the entire amoun, and thus only to increase HIS revenue, he plans of doubling the bills. This is a violation of beneficence and non maleficence.

The psychologist is trying to increase revenue by doubling his bills, that the patient would have to pay as these are not covered by the health insurance.

According to me, Dr. Johnny Dee is wrong, as he is not able to do justice to his patients. Though he is not doing any harm on the basis of his treatments, but he is doing wrong by trying to extract more money from the patient. In such a case, he does not maintain his Fidelity, as if someone gets to know of these practices people would not want to continue to come to his clinic for any kinds of treatments. Also, he is causing harm to the patients thus practicing maleficence (harm to the patient) and non beneficence (no benefit to the patient). He is not carrying out justice as the extracting money by wrong means does not fall in that category. Hence, justice, fidelity, beneficence and non maleficence, ethical principles, are all violated.
