The client is a 29-year-old single male. His parents live in Canada and divorced when he was a teenager. He does not keep in contact with his parents and does not disclose his family life. He does not have any siblings. In the last 7 years, he has been in a relationship with a woman who is 10 years older than him. His long term partner is a therapist. The client mentioned several times that he has not been satisfied with this relationship. However, he chooses not to disclose his relationship problems. He is a medical student; however, he failed many courses and is in the process of being dismissed from the medical school. He works part time to pay for some of his daily expenses. He has problems at work as well and often changes his job. His partner supports him financially. He is a heavy drug user, using mostly heroin. He also abuses anxiety medicines such as Xanax. He is under a methadone treatment, and is aware that he is not allowed to use methadone with anxiety pills. He is in the denial phase and does not accept that he has a severe addiction problem. He uses intellectualization defense mechanisms in sessions with his therapist/social worker.

Your Goal:

This is your second meeting ( after intake) with this client. Your goal for this session is to build an alliance with a severe drug user who denies his problems, is smart, arrogant, and ignores the therapist’s professionalism and knowledgeability.
