(a). Class C pharmacy records must be kept for at least 2 years from the date of record.

(b). These informations are required on an original medication order:- date and time, medication name, dosage strength and form, administration route, indication of use, frequency of administration.

(c). Informations required in a PMR are :- name, date of birth, address, pharmacy name, emergency contact details, name of Current medication, dosages, frequency, reasons of treatment, details of allegries of any, medical history of the patient.

(d). Children under 14 will be disclosed information in presence of their parents. From the age of 14 to 18 can request not to disclose their information even to parents. For paitents above 18 years of age all Medica information are confidential and belongs to the patient. If the patient is incompetent to handle his own information, the appointed representative can have access to the confidential information.
