Mr and Mrs Warriesome became concern about thelr 4 year old son, Tom, when they noticed that he was notable to manipulate his environment nor was he socializing like his Pre-K friends For example, he was usually clumsy and would often fall for what seems like no reason. He had difficulty sitting, standing and has developed a waddling walk (gait) at times. He would grab things form his friends and sometimes even bite them. Tom has also complained, increasingly, about stomach cramps and has sometimes refuses to eat. This of course has led to some weight loss. These changes were becoming more pronounced so much so, that they became apparent to Tom's teachers. The teachers met with Tom's parents to share their observations. They were most concern that Tom, had difficulty climbing stairs and the Monkey bars on the playground. They explain that he would sit and then use his hands and arms to climb most of the way. They were also afraid he would hurt himself or the other students, so he has to be monitored constantly. The teachers made reference to the fact that Tom rarely eats his lunch because of what he describes of stomach aches. After the meeting, the Worrisomes decided it was time to take Tom to his pediatrician, Dr. Childs. Before they were able to make their appointment date, Tom developed a nasty respiratory infection, which was later diagnosed by Dr. Childs as pneumonia. The pediatrician did a detall history of the events and symptoms. He then requested some basic tests. When the results came back, Tom's pediatrician referred him to neurologist, Dr. Foot. Dr. Childs indicated then that Tom might eventually have to see, a cardiologist, pulmonologist and physiotherapist. Dr. Foot immediately noticed Tom's poorly developed muscles. He then ordered some additional neurological tests. A week later, Dr. Foot explained to Warriesomes, that Tom's symptoms, test results and his obvious muscles wasting and weakness were all pointing him towards one diagnosis. Using your knowledge of skeletal and muscle systems, explain clearly, accurately and briefly: 1. What do you think is the "one diagnosis" that D. Foot wil be glving to the Worrisomes? Explain your choice. Your answer should be logical accurate and supported by Information provided in the case study 2. List and explain any two systems (other than muscular and skeletal) that will be directly affected by this disease 3. Answer 1-- From all the sign and symptoms given in the case study one diagnosis can be made and that is Duchenne Muscular dystrophy(DMD) . I support my diagnosis after studying the case very carefully. This disease start at the age of 4 years and causes muscle weakness and later on other organs are also involved.
Answer2 - Nervous system and respiratory system are the other two systems that are affected by DMD. By affecting nervous system, the intelligence of the the child affected. The child progress to mental health problems. Due to muscle weakness, Respiratory muscles are not working properly and breathing problems and infections are resulted.
Answer3 - Tom will have to see cardiologist, pulmonologist and physiotherapist because of the complications and infectitious involvement of other organs like muscle weakness, Respiratory muscles weakness, lung infection, skeletal muscle involvement, heart muscles involvement, difficulty in walking, poor brain development, congestive heart disease etc.
Answer 4- Since DMD is genetic disorder, so not many drugs will help. Corticosteroids may have some beneficiary effect. There are symptomatic treatment for the disease. Physiotherapy and orthopaedic appliances are also used to help walking. For lung infection treatment, drugs according to the infection can help.
