Textbook source for this question would be great

Following an illness due to campylobacter infection, a young University student presents to his GP with weakness in his limbs and absent reflexes including the knee-jerk reflex. Sensory testing in his feet to light touch, pressure, vibration, and temperature were conducted. In addition, a sample of cerebrospinal fluid was taken. The patient was provisionally diagnosed with a disease of the peripheral nervous system affecting production of myelin a) b) c) d) What is myelin? Provide a brief description of its composition and function. How is myelin produced in the central and peripheral nervous systems? Include the specific cells responsible Provide the rationale for the sensory testing. What would you expect the findings to be? (2 marks) (1.5 mark) (3 marks) Using a diagram, provide a detailed description of how the knee-jerk reflex works. Why was the knee-jerk reflex absent in the patient? What is cerebrospinal fluid (CSF)? Where is CSF usually sampled from and why? How does CSF sampling help support the diagnosis of a demyelinating disease? (1.5 marks)

Explanation / Answer

Answers-

A. Myelin is a white fatty substance which covers and surround the some of the axon of neural cell and provides the electrical insulation effect. Myelin is formed by the process of myelinogenesis and cells responsible for myelin formation are- for central nervous system is oligodendrocytes & for peripheral nervous system is schwann cells.

B. Rationale of sensory testing -

It is to be performed for confirmation of responses from various type of triggers like pressure, pain, temperature, electric, vibration & light by providing various stimuli. Doctor expects to get a responces from every stimuli.

D. CSF is a clear body fluid found in brain & spinal cord.

CSF samples are taken from spinal cord via sample collecting process called lumbar puncture. CSF sampling helps to diagnose the demyelinating disease by screening of sample for any kind of infections and could build a culture for proper diagnosis.
