Question

Susie Anthony, a 19-year-old patient, is seen today for a general eye examination. The physician orders a routine Snellen test, and Kim administers it. Susie wears contacts. With her right eye, she reads without errors to the 20/25 line; however, she squints and makes three errors at the 20/20 line. With her left eye, Susie makes two mistakes at the 20/30 line. How should Kim document this procedure?

Why did Susie leave her contacts in? How does Kim need to make sure that Susie does not squint and lean forward? How will you try to arrange this is your own practice with your particular doctor? Answer the question posed at the end of the paragraph in the book. Write 5 paragraphs.

Explanation / Answer

The sharpness of one’s vision testeted through the process of Snellen’s eye testing and it is also called as a visual acuity. During the process of these testing different sizes of letters contained white chart placed on board at certain distance from the person. The distance at which the person reads letters normally (emmotropic vision) is printed at the end of that line. In Snellens test both eyes are tested separately, because both eyes do not have same vision, as two eyes have two Individual sets of processes for vision. The vision of one eye could be different from the other one.

Kim can document the given results as given below:

X/XX/XX (date) X:XX AM/PM (time) Visual acuity completed c Snellen chart. OD 20/25, 20/20-3, OS 20/30-2 c corrective lenses. Squinting present. Kim Tau, CMS (AAMAN).

In the above documentation, “OD” refers to the right eye and “OS” refers to the left eye.
