Because when she transfuse both blood and dextrose in same iv set than due high osmolarity of 2% dextrose in compare with 0.9% saline Rbc get shrink because of mix with hypertonic solution. This lead to lysis of rbcs and haemolysis occur and osmotic fragility increased.
