The close out form currently contains basic information about the project presenters and the evaluators, which is mandatory to be present in the closeout form. Apart from these, it only has the objectives of the project in the close out form. Though the objectives of the project is also important to be present in the close out, i feel that there should more than just the objectives of the project in the close out form.
The close out form should ideally give you an overview of the project, without having to go through the entire project report to understand what the project intends to do. To achieve this purpose, in addition to the project objective, i would suggest including the project synopsis in the close out, which gives an overview of the project implementation.
Also, the close out should include the key findings of the project so that the reader gets a comparison between what the project set out to achieve (Objectives) versus what it actually achieved. (Key findings).
It can also include acknowledgments, where in the key contributors to the project can be acknowledged.
Ultimately, the close out should be in such a way that it gives an overview of the project and it induces interest in the reader about the content of the project.
