b) The root cause of the incident was a faulty side valve that allowed the catalyst barrier to slip down followed by allowing hydrocarbon gases to enter the air side of the FCC which caused a build up of pressure

c) Recommendations offered by the CSB include inspection of all critical components like side valves from time to time to check for wear and tear, complete shut down of ESP so as to eliminate any possible ignition of gases as well as checking the adequate level of steam for flushing the fractionating column

d) The hazard was obvious before the event, therefore there were checks like the catalyst barrier to ensure that the hydrocarbon gases do not flow through to into the air side of the FCC.
