A) I would give following permissions:

Set time for a given interval

Set interval of snooze

Select duration of alarm

These are chosen because ther are the most basic and effect most people . Also simplicity of these would help.

b) I would like to add customizations as reminders or notes within the app to tell people about any important commitments so that they can set alarm accordingly.

c) Additioinal permissions may help like adding tasks such as taking a selfie to stop the alarm as they would ensure that the user actually wakes up. Fingerprint scanning utility may also help.
