(a) One computer with two different IP addresses at the same time. Yes, multiple IP's can be configured though it is not advisable unless you have multiple NIC.
(b) A data frame with both IP header and ICMP header. yes, ICMP header starts after the IP header and has protocol number 1.
(c) A data frame with both IP header and ARP header. YES
(d) Two computers in the same network of exactly the same IP address. YES when seen from external network, but internally they have to have different IP's
(e) Two computers in two different networks of exactly the same IP address. YES If network is internal then two computer can have same local IP
