Consider the McDonald's case below: McDonalds' stated goal is to "Be the world's best quick service restaurant experience." For our purposes, this is a Mission Statement. The next level of goal is a Strategic Goal (can have more than one obviously, but all must support the mission!): "Get the meal out the window 90 seconds from when they have finished placing their drive-through order." Tactical Goal (must help achieve the strategic goal): Explore new technology to predict customer orders, develop timing standards for assembling the five most popular meals, invest in automatic drink-dispensers, and review staffing needs during the lunch hour. Operational Goal(must support the tactical goal): Hire two additional workers for the 11:00 a.m. - 2:00 p.m. time period. Conduct timed training trials for the five most popular orders. Is this action visible when someone goes to McDonald's? Are they attaining their goals? Do they have enough help on board in a typical day? (Operational)

the process starts with taking an order from the customers and ends with serving them the same order. The total tracking process of order do not visible to any one/to the outside customers. the order taken by the server, directly pass to the counter manager, from there to the chef with the same expectations. Hence, all this process may take 90 seconds, but it is not visible to the customers.
Yes, they are attaining their goal most of the times. In few cases, where the order is more customized, irregular, then it takes some additional time to prepare the dish. But the same will be inform to the customers by ther servers. In this case they said the approximate time to serve the food item to the customers. The customers may not worry about the delay of the serve for few seconds to minutes, but they never compromise at quality of the food.
All the process is about coordination and cooperation among the workers, and they are achieving it. Untill these workers are responsive and proactive, these targets can not be achieved. Hence, these workers are well equip with these and they can achieve their goals on any given day I think.
