a) Do you consider yourself handicapped in any way?
This question is a violation of the title "Americans with disabilities act of 1990 (ADA)
This question can be asked as - "Can you perform the specific duties of the job?"
b) Is there a history of chronic illness in your family?
Questions under family health problems are illegal under the ADA. Such questions should be avoided.
c) Are you planning to start a family soon?
This question usually works against the women. And as per human rights commission, this question should be avoided.
d) How does your military experience relate to the job?
This question can be asked. This is an acceptable form of a question, asking how is his previous experience relates to the job he is applying.
e) We are looking for seasoned professionals; you just graduated from college?
This question is acceptable and can be asked. When an employer is looking for experienced candidates and if the candidate applying for the job is a college passout. There is no offence in the question and it can be legally asked.
f) Would you mind if i call you by your first name ?
This question is legally acceptable.
g) Given that you are in a wheelchair, how do you think you'll be able to do this job?
This question is a breach of law, questioning about the diasbility. This question is not acceptable. Such questions on disability should be avoided

Age discrimination is indicated here. such questions should be avoided
I) Are you a smoker or non smoker?
Discrimination against smokers is not illegal in most states, although there is a considerable legal action on this topic.
