Scenario: The RN is caring for a group of patients with two individuals who present a challenge in developing a therapeutic relationship. One is a 15-year-old male who experienced a head injury after being thrown from the back of an all-terrain vehicle (ATV). The patient was placed on a ventilator. On the third day of hospitalization he begins to be more responsive, opening his eyes on request and following the movement of people in the room. The other patient is an elderly woman who does not speak English as her primary language. She had just received a diagnosis where she will need to make serious decisions about her care. Her adult son wants to be her interpreter. There are some very personal questions the RN will need to ask and is uncertain about the son’s involvement.

Describe how the RN establishes a caring, therapeutic relationship with these patients.

Can the RN approach these situations in the same manner as with patients who are awake, alert, and speak the same language?

What strategies may the RN use to provide culturally sensitive nursing care that reflects the patient/family health- illness beliefs and healthcare practices?

RN establishing a caring , therapeutic relationship with these patients ;

Communication is the corestone of nurse patient relationship which is patiened centered care , In which nurse shouls take care multiple factors including the physical conditions ,emotional state ,cultural preferences ,including the patients physical conditions .the timing of the communication is also important when working with patients .the purpose of the therapeutic relations to support patients ,to promote healing to support ,enhance functioning ,Therapeutic relationship are the same regardless of length of contact respect ,empathy ,active lisenting ,trust and confidentiality .this is caring with defined boundaries .

there will be five phases in the patient nurse relationship;

orientation, identification , Expliotation , Resolution and termination .

orientation ; The patients seeks help and nurse assists the patients to identify the problem and extent help needed

Identifcation ; the pateints relates to nurse from independent ,dependent or interdependent posture and nurse assures the she or he understands meaning of his or her situation

Expliotation ; The patients uses the neurse services and other resources on basis of the his or her needs

resolution ; The patient s need are resolved and more mature goals are achieved

termination ; the patients and nurse evaluvate the progress of the intervensions towards the intended goals ,rieview the goals their time togather and end relationship .
