His blood sample should be checked for any infection. Since he was in a hospital he may have acquired a hospital infection.

The most probable cause for the swelling is bacterial infection of the skin.

Diagnosis is that the infection is caused by MRSA whcih is prevalent in hospitals.

In the nursing care plan, the place should be cleaned carefully and topical antibiotics added to the site to prevent further infection. Also antibiotics are given which can kill MRSA.

Plan of care for the patient - regular cleaning of the area and to keep it free from dust. Since these infections can spread easily to other people, the patient should be careful and the family members should also be careful.
