Law of conservation of matter or mass says that the atoms participated in the reaction are same and also the number of atoms remains conserved and hence the total mass should be conserved . Therefore while writing a balanced equation , we follow the conservation of matter.
As the number of atoms are also conserved.. coefficients are used in order to balance the number of atoms in each side(i.e reactants and products)
Phases are just to give information about the state of the compound. And these are not critical in balancing the equation
Each subscript indicates a unique compound/molecule and hence the subscript may or may not be the same while writing the equation
