(a) A gas scrubber on an oilrig discharged gas periodically such that an accumulation of hydrocarbon gas nearby had occurred. A contractor welder was dispatched to a job immediately adjacent to the area and it being a shift change and personnel unavailable temporarily, the welder decided to commence welding. As a consequence, an incident occurred. Describe briefly: what you imagine was the incident including any hazards existing prior to or during the incident (G) (ii) the consequences of the incident on personnel, equipment or process (iii) emergency procedures put into practice during and immediately after the incident (iv) indications of possible cause(s) of the incident (include any breaches of safety procedures (permits to work, etc.) statutory systems, codes of practice and company safety rules that led to the incident and any other causes or contributing circumstancesExplanation / Answer
(i) Previously before the welding or during the welding, an explosion can take place. As welding creates sparks and the Hydrocarbon gas is already present in bulk in the environment. So as soon as a spark occurs a blast can take place which can be dangerous.
(ii) The personnel should have informed the other in charge about the problem or should have stopped the welder before leaving. Due to this, he could lose his job and the equipment can be damaged. Also, the process also has to be stopped for a while.
(iii) During the welding first, it should be made sure that the environment is hydrocarbon free. Apart from that if they should first check with the infrared sensor to determine the leak. If the accident occurs the people working near it should try to evacuate and the fire alarms should start blowing.
