Buffer is any substance which resists any change in pH. It is composed of weak acid and its salt with strong base or weak base and its salt with strong acid.

So, from above definition :

1) Since NH3 is a weak base, it must combines with its salt of strong acid ie 0.35M NH4ClO4

2) sodium hypochlorite is salt of strong base NaOH and weak acid ie hypochlorous acid, HOCl. So, 0.22 M HOCl would be required to prepare the given buffer.
