Question 7: Grocery store owners in the region refused to take calls from investigators or cooperate with the investigation. As a result, a list of food history eaten by the students and their immunization history were obtained from the parents of these students. Parents of the twenty-four students provided information on the source of foods eaten within the past 2 months as well as the polio immunization history. (Four students, including three cases, did not provide the information.) Investigators examined the occurrence of illness among people who ate different food items. Twelve (92%) of 13 students who ate food from the local butcher became ill. Only one (9%) of 11 students who did not eat meat from the local butcher became ill. All but one of the ill students never received polio vaccine. The vaccinated student is related to and visited the butcher one week after his second oral polio vaccine shot and was helping out in the shop. The relative risk for eating meat from this shop was 10.2 (p-value <0.0001). No other exposures were associated with illness. Parents of case-patients reported that they bought their meat from the butcher’s shop.

Given information is that 24 students were affected. some students are being ill and the cause of illness is never found. Immunity information of the students and the food taken by them within 2 months was taken from the parents of the students by the investigators.

12 out of 13 students ate food from the local butcher became ill

but the one from remaining 11 students who did not eat from the butcher shop became ill.

The vaccinated student is related to the butcher shop and was helping.Given that he visited the shop one week after his second oral polio vaccine shot.

The reasons and the investigation should pass over the points following

1) Oral polio vaccine is not using now a days because the few cases of polio that were occuring (8-10 per year) were caused by the OPV vaccine itself and not the wild virus. So the vaccinated one is working in the shop but he may be polio affected person.Through him also illness passes to others through meat.

2)The immunity power of the given students is not given.May be the immunity power of the students is low then the students can easily affected with the illness through the meat,if the meat is damaged.

3)The butchers shop should be checked for the quality of meat.

4)12students ate meat but the one who didnot ate also affected because he was not taken the polio vaccine. So it may be shown him the illness in the same time.

5)Remaining 11 students information is not given.I think they didnot ate food from the butchers shop and they may be healthy.If they were healthy 70% percent may be occured through meat and the remaining percent depends on the immunity and the health conditions of the students.
