Question

Mr. Eisner, age 43, sustained severe burns while welding a gasoline tank that he had removed from a truck. He had full-thickness burns on his face and bald head and on both of his arms and hands - as well as a mixture of superficial and deep partial-thickness burns on his anterior truck. His genital area, lower extremities, and posterior body were not burned. After assessment and emergency stabilization, Mr. Eisner was transported to a regional burn center for care. 1. Using the rule of nines (see diagram), calculate Mr. Eisner's approximate percentage of total body surface area that was burned. JUSTIFY YOUR ANSWER. 2. Should blisters be expected to appear on Mr. Eisner's bald head? Why or why not? 3. Which of Mr. Eisner's burns is likely to cause him the most pain? Why? 4. Why is Mr. Eisner at high risk for hypovolemia?

Explanation / Answer

1. Mr.Eisner has burns on several parts of his body. The total surface area that has burned will be 9% for head, 18% for the arms and hands, 18% for an anterior trunk. So the total comes out to be 45%.

2. The person is bald but still have the hair follicles. These follicles shrink but don't disappear completely. These when exposed to heat often produces small blisters or folliculitis which occurs in superficial to deep partial thickness burns. This appears because the hair follicle gets damaged and allows pathogens access by the small breaks in the skin.

3. Mr. Eisner has full thickness burns on his face and bald head and has superficial and deep partial thickness burns on his anterior trunk. The partial thickness burns of the patient will be more painful which means the burns of the anterior trunk will be more painful.
